Before you fill this in

The worksheet exists to do the arithmetic of a combined rating in one place, because the combination is not a sum. Ratings are combined under a formula that applies the remaining percentage each time, so adding a second disability never simply adds its number to the first. Doing that by hand across several conditions is where mistakes happen, and a small error at the first step compounds through the rest.

Have your individual condition ratings in front of you before you start, taken from your decision letter rather than from memory, and record them in the order they appear on the letter. The order does not change the combined result, but keeping to the document makes the output easier to check against your award. The worksheet is a planning aid for understanding how a combination was reached; it is not a filing and it does not replace your own award letter.
